Rating my manager survey

We are asking this information to help us collect data about team leaders and managers. Getting meaningful can be difficult, so we would like your help. We will use these answers to help students with case studies and to develop strategies to improve their own leadership and management abilities.
We will NOT ask you to identify yourself OR your manager, so no personally identifiable information is asked or collected. As such, we are looking for your honest appraisal of your manager so the data is a true reflection of your opinion.
All of these questions use a 5-point Likert scale that measures how much you agree with the statement in question. You are welcome to share this form with others if you think they would also like to contribute.
In this form, we are using the term “manager” to mean the person you directly report to.
